Changes to Patient Invitations for Long-Term Conditions and Medication Reviews

We wish to inform patients that from April we are making some changes to the way in which we invite patients for their annual reviews of long-term conditions and repeat medications.

What is the change and what does it mean for patients?

Patients will now receive their invitation and/or be reviewed during the month of their birthday.

Whilst we embed the change, this may mean that your recall is sooner or later than usual.

Why are we changing?

We are hoping that the changes will significantly improve our efficiencies and your patient experience, by reducing the need to contact patients multiple times a year with requests for monitoring tests, measurements and the completion of questionnaires.

Who is affected?

This change will cover the patients diagnosed with a range of long-term conditions, including:

  • Heart
  • Diabetes
  • Respiratory (asthma and COPD)
  • Mental Health
  • Cancer
  • Rheumatoid Arthritis

For patients on repeat prescriptions we plan to align these medication reviews as well.

What happens next?
